  6. Trained but not performing safe work procedures
  7. Not meeting physical demands of task
  8. Not communicating needs, expectations or directions to others at time of service
  9. Rushes through the service
  10. Appears unsure, lacks confidence, fearful
  11. Risk of injury due to late in pregnancy, ill, current or pre-existing injury/ medical condition, fatigue, inexperience
  12. Incorrect clothing (too tight, loose, open toe shoes, high heels, etc.)
  13. Personal Protective Equipment not available, adequate or in use
